BUREAU MEMORANDUM
Subject: Empanelment – Private Hospitals, Exclusive Eye Hospitals/Centers, Exclusive Dental Clinics, Cancer Hospitals/Units, Diagnostic Laboratories and Imaging Centers under Continuous Integration Program, 2017 under CGHS, Kolkata – reg.
As per the guidelines of Ministry of Health and Family Welfare, New Delhi – 2017, available on the website of Ministry of Health and Family Welfare, New Delhi on the above mentioned subject (for the program Continuous Integration Plan 2017) for onboarding new Healthcare Organizations (HCOs), it is hereby clarified that Netaji Subhas Chandra Bose Cancer Hospital (a unit of Himadri Memorial Cancer Welfare Trust), 3081, Nayabad, PO & PS — Panchasayar, Kolkata, West Bengal 700 094 was incorporated under CGHS, Kolkata for a period of 02 (two) years after July 19, 2022 or until the validity of HCOs incorporated in 2014 and later or until the next incorporation, whichever comes first.
Healthcare Organizations (HCOs) who have qualified for incorporation under CGHS, Kolkata have signed the Memorandum of Understanding with CGHS, Kolkata and have submitted all other required documents and agreed to the 2014 tariffs of CGHS, Kolkata uploaded to CGHS website, Kolkata.
A copy of this office memorandum along with a list of incorporated HCOs is placed on the website of CGHS, Kolkata (www.cghskolkata.nic.in).
