When you complete the enrollment process online, you will get a final window pop-up that have your REFERENCE number and your MEMBERSHIP ID number.  If you print that information, you can take the paperwork to your local Ameriplan® dentist and use it until you receive your membership cards and booklet in the mail within 7 - 10 business days upon approval of your application.
